Why Ortho-K Needs an Experienced Orthokeratologist

Ortho-K works like braces for your eyes. Its custom rigid contact lenses gently and gradually reshape the front surface of the cornea — much as Invisalign and dental braces reposition teeth.

Because fitting these lenses takes specialised instruments, training and experience, not every optometrist in Singapore can prescribe Ortho-K. BENEFITS OF ORTHO-K

Ortho-K can correct mild to moderate short-sightedness, long-sightedness and astigmatism, giving you clear, functional vision throughout the day without glasses or daytime contact lenses. We routinely fit Ortho-K for prescriptions up to -7.00 dioptres, and higher prescriptions are sometimes possible depending on suitability — the strongest we have fitted is -8.00 dioptres of myopia.

Ortho-K for Adults

HOW DOES ORTHO-K WORK?

Ortho-K reshapes your cornea while you sleep. Each lens is a precision-designed rigid gas-permeable contact lens; as you wear it overnight, the lens and your tear film apply gentle, even pressure to the front of the eye, temporarily moulding its shape so that light focuses correctly the next day. Every lens is fully customised, with a pre-programmed treatment effect matched to your prescription, eye shape and vision needs. Because they are worn during sleep, most people adapt to them quickly.

The process is safe and completely reversible: stop wearing the lenses for 48–72 hours and the effect gradually fades as the cornea returns to its natural shape. For many people this makes Ortho-K an appealing non-surgical, non-permanent alternative to laser vision correction — and for children it adds the benefit of slowing myopia progression in a way that ordinary glasses and single-vision contact lenses cannot.

No — Ortho-K is a well-established, well-proven method of vision correction. The idea of using contact lenses to reshape the cornea has been studied for around 40 years, and clinics across Asia, including Hong Kong, China and Singapore, have been fitting Ortho-K for well over two decades.

I’ve heard different names for Ortho-K lenses — are they all the same?

Essentially, yes. Dream lenses, sleep lenses, night lenses, eye braces, OK lenses, Paragon CRT (corneal refractive therapy) and VST (vision shaping treatment) are all marketing names for orthokeratology. The fitting systems and lens designs differ from one another, but the underlying principle — gently reshaping the cornea overnight — is the same.

At EMME Visioncare, every Ortho-K lens is individually designed using the latest-generation, fully customisable 3D Ortho-K technology. Compared with older systems that were limited in their lens parameters, this lets us correct more complex prescriptions and eye shapes with more predictable results.

Is Ortho-K safe?

Yes — Ortho-K is a safe form of vision correction when it’s properly prescribed by a trained practitioner and the lenses are worn and cared for as instructed. Like all contact lenses, Ortho-K lenses carry some potential for complications, but they’re made from a breathable, highly oxygen-permeable material designed specifically for overnight wear.

Most contact-lens eye infections, across every type of lens, come down to poor hygiene and poor compliance — not washing hands before handling lenses, rinsing lenses with tap water, showering or swimming in them, skipping proper disinfection, or re-using cleaning solution. Because Ortho-K lenses are removed when you wake, several of these risks (such as swimming or showering in lenses) don’t apply. A diligent Ortho-K wearer is generally at lower risk than a careless wearer of regular low-oxygen soft or cosmetic lenses.

In our own practice, careful candidate screening, precise lens fitting and thorough aftercare guidance are central to keeping Ortho-K wear as safe as possible for every patient.

What is the main risk of Ortho-K, and how common is it?

The main risk of any contact lens, including Ortho-K, is eye infection (microbial keratitis) — but it is rare. Published studies estimate the incidence of microbial keratitis in overnight Ortho-K wear at roughly 5–8 cases per 10,000 wearers each year — comparable to the risk of daily-wear soft contact lenses, and lower than overnight extended-wear of soft lenses.

Importantly, most infections — across every type of contact lens — are linked to poor hygiene and compliance rather than the lenses themselves: not washing hands, rinsing with tap water, or skipping proper disinfection. Following the handling, hygiene and disinfection routine your optometrist recommends is by far the most effective way to keep your risk low. In children, the risk is low and broadly comparable to daily soft contact lens wear, and we monitor every young wearer closely with regular follow-up.

Who is a good candidate for Ortho-K?

Ortho-K suits almost anyone who wants clear daytime vision without glasses or daytime contact lenses. It’s particularly well suited to:

  • People whose work brings on eye irritation or dryness with daytime contacts — long hours at computer screens, air-conditioning, smoky areas or dusty outdoor work sites.
  • People with sensitive eyes or a tendency towards allergies.
  • Those with active lifestyles — fitness enthusiasts and keen swimmers in particular.
  • Adults considering laser correction but whose prescription hasn’t yet stabilised, or who would rather avoid the risks of permanent surgery.
  • Children, who can see clearly for school, sport and swimming without the hassle of glasses — or the risk of breaking or losing them. For short-sighted children, Ortho-K is also one of the most effective options for slowing the progression of myopia.

Can my prescription be corrected with Ortho-K?

Ortho-K can correct most common prescriptions — myopia (short-sightedness), astigmatism (an uneven corneal shape), hyperopia (long-sightedness) and even presbyopia (the age-related difficulty focusing up close).

For short-sightedness, Ortho-K generally works best between -1.00 and -6.00. Higher corrections are often possible depending on your eye parameters and vision expectations, and a partial correction can be a good option for people with very high myopia. Thanks to the latest, more complex lens designs, even moderate astigmatism — up to around -4.00 — can be treated. In our own practice, our experienced optometrist has successfully corrected myopia up to -8.00 with Ortho-K.

Is there an age limit for Ortho-K?

No — there is no upper or lower age limit for Ortho-K. Children as young as five have been successfully fitted, and adults of any age can wear Ortho-K provided their eyes are healthy and suitable.

For children, one of the biggest advantages is that Ortho-K can help slow the progression of short-sightedness as they grow. That matters well beyond convenience: keeping a child’s myopia lower reduces their long-term risk of the sight-threatening conditions linked to high myopia, including glaucoma, retinal detachment and myopic macular degeneration.

Suitability depends on a number of factors, so the best way to find out whether Ortho-K is right for you or your child is a detailed Ortho-K assessment with our optometrist.

How effective is Ortho-K for myopia control?

Ortho-K is one of the most effective optical treatments for slowing myopia progression in children, supported by a large body of international research on overnight lens wear and childhood myopia.

On average, studies suggest Ortho-K can slow a child’s myopia progression by around 50% — roughly halving the rate at which their short-sightedness would otherwise worsen — though individual results vary. As a simple illustration: a child who might have progressed to around -8.00 without treatment could end up closer to -4.00 with Ortho-K. A lower final prescription means better functional vision and a lower long-term risk of the serious eye conditions associated with high myopia.

How does Ortho-K slow myopia progression?

Ortho-K slows myopia by changing how light focuses on the peripheral retina — something ordinary glasses don’t do. While you sleep, the lens gently reshapes the cornea (the clear front surface of the eye) in two ways: it flattens the centre and slightly steepens the mid-periphery.

That change to the peripheral corneal shape alters where light focuses at the edges of the retina, at the back of the eye. This reduces the growth signal that prompts the eye to elongate — and because myopia worsens as the eye grows abnormally long, slowing that elongation slows the progression of a child’s short-sightedness.

How does Ortho-K compare to other myopia control treatments?

Ortho-K is one of the most established optical treatments for myopia control, and it works alongside other proven methods rather than competing with them. Atropine eye drops are a medical treatment that is also very effective for many children. Because Ortho-K and atropine work through different pathways in the eye, having more than one proven approach means treatment can be tailored to how each child responds.

Every child is different — one may do well on a particular method while another doesn’t. Soft myopia-control contact lenses are another route; they don’t correct astigmatism as fully as Ortho-K and their measured effect is slightly lower in some studies, but they remain an excellent option for many children.

For children with more rapid progression, combining treatments can help: recent published studies have shown an enhanced myopia-control effect when Ortho-K is used together with atropine therapy.

Can I wear Ortho-K if I have dry eyes?

Yes — most people can wear Ortho-K even if they have dry eyes. In fact, rigid lenses like Ortho-K can suit dry-eye sufferers better than soft lenses, which tend to draw moisture from the surface of the eye.

Many dry-eye symptoms are triggered by the environment — air-conditioning and long hours at a screen — and these simply don’t apply overnight while your eyes are closed. It’s one reason many of our adult patients switch to Ortho-K after struggling with dryness in daytime contacts. As part of your suitability assessment, we always check your tear film and eye surface to make sure Ortho-K is a good fit for you.

How quickly will I see clearly with Ortho-K?

Most people notice clearer vision after the very first night of Ortho-K wear, with further improvement and stability over the first one to two weeks as the eye settles into treatment. With lower prescriptions, excellent vision can usually be reached within a week; stronger prescriptions take a little longer — you can read more about how quickly Ortho-K corrects vision here.

During this initial adaptation period, our Orthokeratology Fitting Package includes a pair of glasses or soft contact lenses at half strength, so you keep good functional daytime vision while your eyes adjust.

Are Ortho-K lenses uncomfortable to wear?

Most people find Ortho-K lenses comfortable, with only mild awareness during the first few nights. As your eyes get used to them, you may feel a slight sensation after inserting the lenses — a little like having an eyelash in your eye — before you fall asleep. This is completely normal and nothing to worry about.

Most of that sensation comes from your eyelid moving over the lens as you blink. Once you close your eyes to sleep you’re unlikely to notice the lenses at all, and they won’t disturb your sleep. By the end of the first week, most wearers find the lenses settle and feel comfortable.

Can I see while wearing my Ortho-K lenses?

Yes — you can see clearly while the lenses are in. We recommend inserting your Ortho-K lenses about 10 minutes before bed so they have time to settle. You can watch TV or read with them in, and if you wake up in the middle of the night to go to the bathroom, you’ll still be able to see.

How many hours a night do I need to wear Ortho-K?

We recommend wearing your Ortho-K lenses for at least 6 hours each night, and ideally around 8 hours. Consistent overnight wear gives the cornea enough time to reshape fully, for the clearest and most stable daytime vision.

Will my vision stay clear all day with Ortho-K?

For most people, yes — Ortho-K keeps your vision clear throughout the day. Depending on your pre-treatment prescription you may notice a slight fade towards the end of a long day, but for most wearers this isn’t a problem.

When we design your lenses, we factor in a small amount of expected daytime regression so that good vision is maintained for most of your waking hours. With lower prescriptions, clear vision often lasts a full day after a night of lens wear — and sometimes longer.

Do I have to wear Ortho-K every single night?

Not always — once your treatment effect has stabilised, you may not need to wear your lenses every single night, depending on your level of correction and vision needs. Many of our patients can skip the occasional night and still keep satisfactory vision the next day.

For example, children heading off to school camp for a few days can usually see clearly without taking their Ortho-K lenses along, then simply resume wear when they get home to restore the full treatment effect.

How often should Ortho-K lenses be replaced?

We recommend replacing your Ortho-K lenses once a year for the best vision results, comfort and safety. With normal use, microscopic deposits, tiny surface scratches and small changes in lens shape gradually build up — and over time these can reduce how effectively the lenses reshape your cornea.

How long can I keep using Ortho-K?

You can continue Ortho-K for as long as you like. Children using Ortho-K for myopia control generally wear the lenses until their short-sightedness stabilises — usually in their late teens or early adulthood — though many choose to keep wearing them afterwards for everyday vision correction instead of switching to glasses or daytime contacts. Some people have worn Ortho-K happily and successfully for more than 20 years.

Can I stop Ortho-K, and is it reversible?

Yes — Ortho-K is temporary and completely reversible. If you stop wearing the lenses, your eyes naturally return to their original shape over a few weeks, and choosing Ortho-K now won’t prevent you from having laser vision correction later if you decide to.

For children, there’s an added benefit: keeping their prescription lower with Ortho-K while they’re young gives them more options for permanent laser correction as adults, should they ever wish to go that route.

How widely used is Ortho-K?

Ortho-K is a well-established form of vision correction across Asia, Europe, North America and Australia, and adults and children have been treated successfully with it for many years. It has been especially widely adopted in East Asian populations — such as China, Hong Kong and Taiwan — where myopia (short-sightedness) has long been far more prevalent than in much of the rest of the world, alongside greater public awareness of myopia prevention and control.

As myopia rises globally, including here in Singapore, we’re seeing growing interest in Ortho-K from parents concerned about their children’s worsening eyesight who want to do more to protect their long-term vision and eye health.

Why should Ortho-K be fitted by a specialist?

Ortho-K fitting requires specialist training, technology and ongoing care — not every optometrist is equipped to fit it. Because each lens is custom-designed individually for your eyes, the process takes extra time, expertise and experience, which is part of what goes into the cost of Ortho-K.

Successful Ortho-K also depends on close aftercare — regular follow-up visits, ideally with the same practitioner, for continuity of care and the best results. For that reason, Ortho-K is best fitted by a practitioner with a genuine interest and experience in specialty contact lenses.
